Recreation and Social Community Integration Activities
Tennessee Aquarium, Chattanooga Zoo, Tellus Museum, Camps, Equine Therapy, Aqua Therapy, Fitness Classes, Karate Classes among other community activities.
Exploration, development, job coaching, and long-term supports on the job.
Regular check-ups, extractions, fillings, crowns, etc.
Diagnosis and evaluation, nursing services, consultation with other medical/health care providers.
Routine eye exams, eyeglass lenses, non-designer frames, etc.
Ramps, door widening, stair lift, backyard fence, bathroom modifications, as well as other modifications as defined in service plan.
Special Medical Supplies
Briefs, protective underwear, bed pads, mattress covers, weighted blankets/ vests, etc.
Community Living Supports
Assist an individual with learning to clean their home, shop for groceries, pay bills, etc.
Community Access Activities
Assistance with transportation to and participation in community activities
Individuals, ages three (3) and up, with one or more of the following:
- Developmental Disablity
- Intellectual Disability
- Cerebral Palsy
- Ausitm Spectrum Disorder
- Down Syndrome
- Based on need instead of family income
- Up to $3000 per year may be approved for services
- All services must be approved before they can be provided
