Transition Age Youth and Young Adult Supplemental Grant
The Department of Behavioral Health and Developmental Disabilities (DBHDD) has named Lookout Mountain Care Management Entity (LM CME) as a recipient of the 2015 Transition Age Youth and Young Adults (TAY/YA) Supplemental Grant. Money from this grant will be used to support TAY/YA in a way which reinforces their recovery process, thus ensuring a seamless transition into the adult service system. The protocol which LM CME intends to follow will assist furthering self-sufficiency of these transition age youth and young adults. Further, this initiative will demonstrate the effectiveness of High Fidelity Wraparound on transition age youth and young adults with Severe Emotional Disturbance.
During the grant period, a total of 30 youth between the ages of 16-25 will be enrolled in Wraparound through LM CME. We will include two populations of focus: 1) Youth transitioning from Department of Juvenile Justice (DJJ) facility-based placement to the community and 2) Youth seeking employment or enrolling in institutions of higher education, such as technical schools, community colleges, and universities. Referrals for this population will be made by staff and the community.
